Tender Loving Care Aged Care – Bankstown is seeking a Care Partner to support customers through their care journey, coordinating services and developing holistic care plans to help them live safely at home or in a preferred setting.
You will build solid relationships, conduct onboarding assessments, and work with clinical teams and families to ensure person-centred care and smooth transitions across health needs.
