Job Description
Western NSW PHN is seeking a Population Health Coordinator to work with general practices across the Western NSW Primary Health Network region.
The role supports primary care workforce development, education and e-Health adoption, in collaboration with GPs and allied health professionals.
The position involves working from one of our NSW offices (Dubbo, Orange, Bathurst or Broken Hill) with a hybrid-friendly approach, engaging in regional initiatives to improve patient care and service
