Job Description

Mercy Health is seeking a Social Worker/Care Coordinator for the Health Independence Program in Werribee. The role is part time (0.6 EFT) and involves managing complex care, home visits, and psychosocial assessments to support HIP clients.

You will collaborate with a multidisciplinary team, assist clients in accessing Centrelink, NDIS, and VCAT, and contribute to case conferences. Bachelor or Masters of Social Work and relevant checks are essential.
