Job Description
Whiddon is seeking a Community Coordinator for our Tamworth office to lead and develop social, family and community networks for older Australians in our care.
This fixed-term, full-time role supports care plans, assessments and goals to help clients live safely and independently within the community.
You will work with a team-oriented team, manage case coordination, and engage with Home Care Packages, Commonwealth Home Support Program, NDIS and ICare while promoting high standards of service
