Job Description

Hypar Group is seeking dedicated mentors to support participants across Moreton Bay and Sunshine Coast with complex disability, psychosocial needs and mental health support. You will mentor, build relationships, and implement person-centered strategies in line with NDIS practice standards.

Role involves documenting progress notes, incident reports, and collaborating with families and allied health specialists.
