CQ Medicentre South is seeking an experienced Chronic Condition Management Nurse to join our established and highly regarded practice in Rockhampton.

About the Role This position has a strong focus on Chronic Condition Management and preventative healthcare initiatives.

Responsibilities include
Coordinating Chronic Condition Management plans
Conducting Care Plan Reviews
Completing Health Assessments
Patient education and health coaching
Recall and reminder management




Supporting preventative healthcare initiatives
Working collaboratively with GPs and allied health providers
Monitoring patient outcomes and follow-up activities
Maintaining accurate clinical documentation

About You We are seeking someone who is passionate about patient-centred care and enjoys helping patients achieve better long-term health outcomes.

You will have
Current AHPRA registration as a Registered or Enrolled Nurse
Current CPR certification
Experience in General Practice nursing
Demonstrated experience with or the ability to learn: Chronic Condition Management Plans, Care Plan Reviews, Health Assessments, Patient follow-up programs.
Excellent organisational and communication skills
Full Australian working rights
Experience using Best Practice software will be highly regarded.
