Eastern Health is recruiting Case Managers for the Mental Health Clinician Older Persons Community Team across Ringwood sites and Peter James Centre. The roles range from casual to permanent full time/part-time, with on-site parking and café facilities.
You will deliver recovery-based case management in a multi-disciplinary setting and conduct outreach visits to consumers. Successful applicants will have qualifications in OT, SW, nursing or clinical psychology and a strong commitment to
