Job Description
BlueCare, part of the UnitingCare Family, is seeking a Care Partner to deliver person-centred and high-quality care in community settings across Recent Farm, Hamilton, Kedron, McDowall & surrounding areas.
You will work with clinical teams to plan and coordinate services, ensuring best outcomes for clients funded by aged care services.
You will spend most days on the road engaging with clients in the community, supporting care plans, budgets, and linking clients to necessary resources.
