Job Description
HammondCare is seeking a permanent part time Clinical Partner – Nursing to support clients living at home across the Central West, Nepean and Blue Mountains regions. This 0.6FTE role involves travel between Penrith, Hawkesbury, Upper Blue Mountains and Central West.

You will work with an experienced multidisciplinary team, conducting home visits and group activities, delivering person‐centred care and setting realistic care goals to help clients stay active and independent.
