Calvary Health Care invites applications for the Clinical Project Lead – Dementia Model of Care to lead the implementation and evaluation across hospital, residential aged care and home care services.

Working with consumers, families, carers and clinicians, you will drive person-centred, evidence-informed dementia care, oversee project activities, engagement and education, and partner with leaders to embed the model and sustain improvements across Calvary services.
