Bethanie is seeking a dedicated Case Manager to coordinate Home Care for clients across Claremont to Osborne Park and Yokine. You will take a holistic wellness approach, balance client visits with home-based work, and collaborate with nurses, allied health staff and support teams to deliver personalised care.
The role is permanent, with standard business hours and at least one onsite day per week at our Gwelup office.
