About the Role
n
Carbal is seeking an Aboriginal Health Worker or Aboriginal Health Practitioner to join our Toowoomba Clinic team.
n
You'll play an important role in delivering culturally safe, person‐centred primary health care and helping clients access services, engage with their care and achieve better health outcomes.
n
Working alongside GPs, Nurses, Allied Health Professionals and other support services, you'll contribute to health assessments, chronic disease management, care planning, health promotion, clinical support and community engagement.
n
You'll also help clients navigate the health system, build trusted relationships and ensure culture and community remain central to the care we provide.
n

What You'll Be Doing
n
Clinical Support
n
n
Assist with health assessments, including 715 Health Checks
n
Support chronic disease management, care planning and follow-up activities
n
Assist GPs and Nurses with clinical procedures and treatment room activities
n
n
Client Engagement & Cultural Support
n
n
Build solid relationships with Aboriginal and Torres Strait Islander clients and community
n
Support clients to access health services, attend appointments and navigate the health system
n
Promote culturally safe, respectful and person-centred care
n
n
Health Promotion & Community Engagement
n
n
Support health promotion, education and preventative health initiatives
n
Encourage healthy lifestyles and early intervention
n
Participate in community engagement activities and programs
n
n
Teamwork & Clinical Administration
n
n
Work collaboratively as part of a multidisciplinary health care team
n
Maintain accurate clinical records and documentation
n
Use clinical software systems to support quality care
n
Contribute to quality improvement, accreditation and continuous improvement activities
n

You'll Be a Great Fit If You...

Identify as an Aboriginal and/or Torres Strait Islander person
n
Hold, or be nearing completion of, a Certificate III in Aboriginal and Torres Strait Islander Primary Health Care
n
or
n
Hold a Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Health Practitioner)
n
If joining as an Aboriginal Health Worker, be willing to continue developing your skills and qualifications where appropriate
n




Have experience delivering primary health care services or a strong desire to build your career in Aboriginal and Torres Strait Islander primary health care
n
Have a strong understanding of health issues impacting Aboriginal and Torres Strait Islander people and communities
n
Be able to build positive relationships with clients and community
n
Have strong communication and interpersonal skills
n
Work effectively within a multidisciplinary team
n
Be organised and able to manage competing priorities
n
Hold a current Open C Class Driver Licence
n
Hold current CPR certification
n
Hold, or be able to obtain, a Blue Card and National Police Check
n
Experience within an Aboriginal Medical Service or Aboriginal Community Controlled Health Organisation
n
Experience supporting chronic disease management and health assessments
n
Experience using Best Practice or similar clinical software
n
Additional training in health promotion, preventative health or community engagement
n
Previous experience in health, aged care, disability or community services
n
