Our Ozcare Aged Care at Home Branch in Chermside is looking for a full-time Care Partner to add value to their team. If you are a compassionate individual who can help support our older generation in living independently then this role is for you.

Your Role
n
You'll be responsible for planning, managing and leading client care and support across a range of programs. You'll also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care.

As a Care Partner you will:
n
n
Manage and coordinate care services to ensure the highest standards of care
n
Develop and implement care plans tailored to individual client needs
n
Liaise with clients, families, and healthcare professionals to ensure comprehensive care
n
Foster robust relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals
n
Ensure compliance with all relevant legislation and organizational policies
n
Monitor and evaluate care services to continuously improve quality and efficiency
n
Monitor the budget of Home Care Packages and ensure clients understand their funding and fees
n

For Success you need to:
n
n
Demonstrate experience in case management and client care coordination in a Community Aged Care setting
n
Have demonstrated solid verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
n
Be vaccinated against Influenza & at least 1 dose of Covid-19 vaccine
n
Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
n
Hold a current Police Screening Certificate
n
Provide a copy of your current First Aid and CPR Certificate
n
Hold a current Queensland driver's licence
n
Completed tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services)
