You will build trusting relationships, develop tailored care plans, and deliver person-centred care in line with budgets.
This part time, permanent role is 56 hours per fortnight, paying $* per hour plus super, salary packaging and employee discounts.
Occasional face-to-face visits or video check-ins help maintain connection.
