Job Description

Heathcote Health is seeking a compassionate Social Worker to deliver services across acute, aged care and community settings. You will work in a multidisciplinary team to support clients, families and carers through assessment, counselling, advocacy and care planning.

The role involves short-term counselling, developing person-centred care plans, and contributing to quality improvement. A recognised Social Work degree and AASW eligibility are required, with a current driver licence and required
