Job Description

We are seeking a reliable and compassionate Enrolled Nurse (EN) to join our community home care team. This position will be primarily based in our Geelong office, with travel to clients across Geelong and Ballarat as required.

The role is ideal for an Enrolled Nurse who enjoys a combination of office-based clinical documentation and community client visits and is looking to develop experience within the home care sector.

Key Responsibilities

- Complete client care plans and reviews plus necessary documentation for onboarding clients.
n
- Conduct client assessments and reassessments within EN scope of practice.
n
- Complete home and community-based client visits across Geelong and Ballarat.
n
- Gather and document relevant clinical information for review by the Registered Nurse.
n
- Prepare clinical documentation requiring RN review/countersigning where applicable.
n
- Complete progress notes, risk assessments and other required clinical documentation.
n
- Identify changes or concerns regarding a client's health or care needs and elevate these to the RN.
n
- Assist with maintaining accurate and up-to-date client records.
n
- Communicate professionally with clients, families, care staff and the clinical team.
n
- Work collaboratively with the RN, Care Coordinators and other members of the team.
n
- Follow organizational policies, procedures, infection-control requirements and clinical governance processes.

About You

n

n
- Current AHPRA registration as an Enrolled Nurse (EN).
n
- Minimum 2 year of nursing experience.
n
- Good clinical assessment and nursing skills.
n
- Community, home care or aged care experience is highly desirable.
n
- Able to complete client assessments, care plans and clinical reviews.
n
- Able to recognize changes in a client's condition and escalate concerns to the RN when required.
n




- Understands EN scope of practice and works under appropriate RN clinical oversight.
n
- Excellent time-management and organizational skills.
n
- Able to prioritize workload and meet documentation timeframes.
n
- Accurate and confident with clinical documentation and progress notes.
n
- Good communication and interpersonal skills.
n
- Able to work independently as well as part of the clinical and office team.
n
- Reliable, skilled and compassionate.
n
- Good computer skills.
n
- Current driver's license and willingness to travel between Geelong and Ballarat as required.
n

Important

This position is primarily an administrative, reception and rostering coordination role. It does not involve independent clinical decision-making or case management responsibilities.

The successful applicant will be expected to work within their delegated authority and elevate clinical, HR, financial, compliance, client-risk or other complex matters to the appropriate manager.
