NSW Health is seeking a Registered Nurse to join the Drug Health Assertive Community Management Team at Mt Druitt Centre for Addiction Medicine. This permanent part time role provides assertive, community-based nursing care and evidence-informed case management to diverse communities.

You will assess, plan, implement and evaluate individualized care while building therapeutic relationships with patients, carers and the multidisciplinary team, and you will maintain thorough clinical documentation.
