Job Description
HealthAbility invites applications for a Health Navigator – Pathway to Positive Health in Melbourne. This fixed term, 0.6 FTE position provides multidisciplinary community health support, focusing on vulnerable children and families.

As Health Navigator you will conduct health assessments, triage, coordinate referrals, advocate for clients, and collaborate with a range of health and community services to improve outcomes.
