Support Partner - Help Older Australians Stay Independent (Hybrid Role)

South Melbourne | Full Time | 3 days in office

Whether you've been a Care Manager, Case Manager, or Coordinator, if you understand Home Care Packages, particularly Self Managed, and like building relationships - we'd love to hear from you.

What you'll be doing

- Supporting older Australians to stay independent at home
n
- Working with customers and families to build easy, practical care plans
n
- Helping people understand and use their Home Care Package funding
n
- Running Telehealth check-ins, reviews, and support callsKeeping things organised, compliant, and running smoothly behind the scenes
n

What we're looking for

n

n
- Around 1+ year experience in aged care (Care Manager, Case Manager, Coordinator or similar)
n
- A good understanding of Home Care Packages
n
- Comfortable using systems and typing while talking on calls
n
- Someone calm, organised, and easy to talk to
n
- Most importantly - someone who genuinely cares about helping people
n
