HiTalent Recruitment is seeking a Community Mental Health Nurse to join a well-respected and reputable not-for-profit organisation in Bondi, that offers crisis support, mental health services and homelessness support in Sydney. This role offers salary packaging advantages, supervision and support. This is a full-time role, working business hours Monday-Friday.

This community-focused role delivers recovery-oriented outreach and community based supports across psychosocial programs such as the Bondi response team, AOD services and the National Disability Insurance Scheme (NDIS).
n
You will work closely with participants, psychiatrists, families, carers, and the broader community to provide holistic, strengths-based mental health support that promotes independence, stability, and wellbeing.
n
You will be responsible for:
n
n
- Conduct comprehensive mental health assessments and regular wellbeing check-ins
n
- Provide psychosocial supports focused on recovery and empowerment
n
- Undertake case management, including care coordination and service navigation
n
- Monitor and check medications in collaboration with treating psychiatrists and GPs
n
- Develop, implement, and review individualised support plans aligned with psychosocial and NDIS frameworks
n
- Liaise with psychiatrists and other community stakeholders to ensure integrated care
n
- Advocate for participant choice, control, and community participation
n
- Maintain accurate clinical documentation and compliance records
n
n
To be successful in this role, you will require:
n
n
- Current AHPRA registration as a Registered Nurse
n
- Postgraduate qualification in Mental Health Nursing (or working toward) highly regarded
n
- Demonstrated experience in community mental health
n
- Strong understanding of psychosocial disability and recovery-oriented practice
n
- Experience working within the NDIS framework
n
- Excellent communication and stakeholder engagement skills
n
- Current NSW Driver's Licence
n
