Job Description

Better Health Network is seeking a passionate Case Manager - Complex Care to join our Pre-Housing Homelessness Program. Based at the Victorian Pride Centre, St Kilda, you'll walk alongside people experiencing homelessness as they navigate health, housing and community supports toward stability and housing outcomes.
