Monash Care Disability Services – Oakleigh VIC

- Assistance with Daily Living (ADLs), including showering, dressing, grooming and personal hygiene
- Meal preparation and assistance with eating where required
- Medication prompting and support in accordance with care plans
- Household tasks such as cleaning, laundry and maintaining a safe home setting
- Community access, shopping, appointments, recreational and social activities
- Transporting participants to appointments and community activities
- Supporting participants to develop and maintain independent living skills
- Providing companionship and emotional support
- Following individual support plans, behaviour support strategies and participant preferences
- Completing progress notes, incident reports and other required documentation
- Communicating effectively with participants, families and the broader support team
