Mercy Health in Werribee is seeking a Social Work/Care Coordination skilled for the Health Independence Program (HIP). The role operates Monday to Friday in a dynamic office with multidisciplinary teams supporting HIP clients across the Wyndham catchment.

Based in a high-performing team, you will conduct comprehensive assessments, home visits, and crisis interventions, while coordinating services and facilitating family meetings to ensure patient-centred care.
