Amplar Health is seeking a Care Integration Nurse to deliver patient-centred nursing care in patients' homes, following care plans, and to undertake clinical reviews within a multidisciplinary team.

You will visit patients in the community, using the company vehicle from the Eastwood office, and work a 7-day roster across two shifts (7am-3pm or 2pm-10pm). The role requires flexibility and a compassionate approach.
