Our service-for-one at Palm Cove, is losing one of our valued nursing staff. Our multi-skilled team supports a gentle man with physical disabilities and complex health needs. They deliver an organized, holistic, person-centred support program. Each shift combines nursing care, therapies as recommended by our allied health practitioners, domestic support and activities that fit our man's interests.

This role would suit an experienced nurse who appreciates the opportunity to deliver a high standard of patient care, enjoys developing the skills of non-medical staff and supporting long term caregivers.

Values that support in-home over institutional care would be an advantage. Your client’s home is a calm environment; his purpose-built ‘zen space’. His support program has been carefully built around his individual needs, incorporating recommendations from experienced practitioners and activities that are meaningful to him.

Bring your clinical skills, empathy and capacity for staff training. Provide a supportive leadership role on a team that provides holistic, individualized care to a medically vulnerable person.

Our man needs a good human for this role. A person who can commit to learning about his individual needs, maintaining high standards of care and supporting his team, in a friendly, slower paced environment. A detailed handover and orientation will be provided by our outgoing RN and key members of our care team.

Qualifications and Requirements

- Current registration with AHPRA
- Confidence in your nursing assessments & skills




- Ability to provide leadership on the health aspects of our person's care
- Capacity to provide clear, step-by-step written instructions to the non-medical care team when care needs change
- The ability to follow tasks through to completion, eg. taking responsibility for the completion and management of staff training records
- The ability to work collaboratively with key staff and our man's legal guardian
- The ability to take decisive action in a health emergency

Key Responsibilities

The role involves providing care and support, health management, staff training, clinical administration, domestic support and therapies as recommended by our allied health professionals (eg. gentle physio, hydrotherapy, communication support).

Clinical Administration

- Provide step-by-step written instructions to our support team when care needs change.
- Organize appointments, pathology samples and recent medications as required.
- Manage telehealth appointments and home visits as needed.

Attend medical appts and provide a written record for the team & legal guardian.

- Maintain accurate records of training delivered to individual team members.

Training and Supportive Supervision

- Hand hygiene and infection control
- Giving and recording medication
- Taking and recording vital signs




- Pressure wound prevention
- Complex bowel care (training provided by our outgoing RN)
- Basic wound care
- Catheter care when required.

Liaise with Medical Practitioners & Key Family Members

Our man has a great GP, a private Rehab Consultant, and a full allied health team. There will be opportunities to consult with our man’s parent/guardian regarding his health history.

Provide Person-Centred Care

- Daily nursing assessments
- Nursing care in line with our person-centred approach
- Therapies as recommended by our allied health practitioners
- Support for medical appointments
- Domestic assistance (eg. simple meal prep and laundry)

Hours

We are looking for a nurse from the northern suburbs, who has capacity to work 3 - 4 shifts per week that fit with our roster. The successful candidate will also be required to participate in fortnighly meetings that are relevant to their role.

Mornings 7am to 2pm or 7.30 - 1.30

Afternoons 1pm to 7pm

The capacity to work a day shift on Saturdays would be an advantage.

Conditions

The rate of pay will be negotiated with the successful candidate on the basis of the relevant award, their qualifications and experience. We are looking for a local nurse who has the capacity to attend fortnightly meetings relevant to their position as well as working directly with our man, providing care and other supports as previously mentioned. This is an active hands-on role, that requires a reasonable level of fitness.
