Community Home Care Services is seeking a compassionate, experienced and dedicated Registered Nurse (RN) to join our team in Chester Hill.

This is a full-time fixed-term position providing maternity leave cover for an initial period of 1–2 years, with the possibility of extension/ fix term position subject to organisational requirements and performance.

This is a full-time opportunity for a nurse who is passionate about providing high-quality, person-centered care and supporting clients to remain safe, independent and comfortable in their own homes.

Job Description

As a Registered Nurse, you will be responsible for providing professional nursing care to clients in their homes and community settings. Your key responsibilities will include:

Conducting comprehensive nursing assessments and developing individualised care plans.

Providing high-quality clinical care in accordance with each client's needs and care plan.

Monitoring clients' health, wellbeing and changes in clinical condition.

Administering medications and providing medication management as required.

Wound care, continence care and other nursing interventions within your scope of practice.

Liaising with GPs, allied health professionals, hospitals, families and other healthcare providers.

Maintaining accurate and timely clinical documentation and progress notes.

Supporting clients to achieve their health, wellbeing and independence goals.

Providing education and guidance to clients, families and carers.





Identifying and escalating changes in a client's condition or potential health and safety risks.

Ensuring all care is delivered in accordance with relevant legislation, skilled standards, organisational policies and procedures.

Contributing to continuous improvement and maintaining a high standard of client-focused care.

What We Expect From You

We are looking for someone who is:

A Registered Nurse with current AHPRA registration.

Experienced in community, home care, aged care or a similar healthcare environment.

Compassionate, respectful and committed to person-centred care.

Organised, reliable and able to manage competing priorities.

Professional and committed to maintaining client confidentiality and dignity.
