Firstchance is seeking a Coordinator of Supports to work with families of children and young people with disability, helping them navigate NDIS plans and connect with supports. The role focuses on strengthening families’ capacity to access services and achieve goals within a team-oriented, multidisciplinary team.

Based at our Mayfield office with travel across Newcastle, Lake Macquarie and Port Stephens, you will build relationships with families, providers and communities, supporting
