Job Description

Amplar Health is seeking a Senior RN Care Partner to provide advanced clinical oversight and lead a team delivering person-centred home-based care across a portfolio of consumers with varying needs.

You will mentor Enrolled Nurse Care Partners, guide care planning and risk management, and collaborate with multidisciplinary teams to optimise outcomes and independence for older Australians living at home.
