Health N Home is looking for an organised and approachable Service Coordinator to join our East Brisbane team three days a week.

n

We provide a range of services, including continence assessments, community nursing, disability support, DVA services, NDIS, aged care and home support, and healthcare consumables. As a Service Coordinator, you'll help people access these services and keep the arrangements running smoothly.

n

This is a busy, hands‐on office role. You'll work with clients, participants, families, referrers, nurses, support workers and the wider Health N Home team. You don't need a clinical qualification, but you do need to enjoy speaking with people, managing details and following things through.

What you'll be doing

n

n
- Responding to enquiries and new referrals across our services
n
- Booking continence assessments, nursing visits and other appointments
n
- Coordinating staff diaries, travel time and follow-up work
n
- Keeping clients, families, referrers and staff informed about bookings and next steps
n
- Preparing and following up service agreements, quotes and approvals
n
- Maintaining accurate client, referral and appointment records in Zanda and other systems
n
- Helping with healthcare consumable enquiries and orders
n




- Following up outstanding information so referrals and bookings keep moving
n
- Raising scheduling, service or client concerns with the appropriate team member
n

What we're looking for

n

You'll be someone who can bring structure to a busy day while remaining warm and helpful with the people you speak to. Ideally, you'll have:

n

n
- At least 1–2 years' experience in administration, scheduling, rostering, customer service or service coordination
n
- Clear and professional phone and email communication
n
- Strong attention to detail and reliable follow-through
n
- Confidence learning recent systems and processes
n
- The ability to manage competing priorities and know when to ask for help
n
- Full working rights in Australia
n
- An NDIS Worker Screening Check and Police Check, or willingness to obtain them
n

n

Experience in healthcare, aged care, disability or community services would be helpful, but it isn't essential. We'll provide training in our services and systems.
