True Care Collective is seeking an organised, proactive Service Delivery Coordinator to manage a caseload of NDIS participants and coordinate day-to-day supports across SIL, in-home and community services.
You will work with families, Support Coordinators, allied health professionals and frontline staff to ensure secure, consistent delivery aligned with each participant's needs and NDIS plan, reporting to the Disability Services Manager.
