Job Description

Central Highlands Rural Health (CHRH) is seeking a Care Partner to join our Home Support Program across Macedon Ranges and Hepburn Shire. You will coordinate services, support goal achievement, and provide a person-centred care experience that enables clients to live safely and independently at home.

This is a permanent, part time position with hours to be negotiated. Salary is $48.87 per hour, plus superannuation, reflecting the value of compassionate, community-based care within an integrated
