Job Description

Mounties Care Home is seeking a dedicated Care Partner to join our team on a full time basis at our Hope Island, QLD office. You will coordinate the delivery of care and services to our clients and have oversight over our team of Carers, ensuring quality services are delivered.

The ideal candidate will develop and coordinate client care plans, assist with budgets, support access to My Aged Care, visit clients in the community, and participate in after-hours on-call rosters while maintaining
