Holstep Health in Preston is seeking an Intake and Assessment Worker for fixed-term parental leave cover. The role focuses on holistic assessments, developing goal-directed care plans, and coordinating with carers to improve their quality of life and strengthen caring relationships.

Work pattern includes evening shifts 12:00pm–8:00pm on Wed and Thu, and standard hours Mon Tue Fri 9:00am–5:00pm, with hybrid arrangements available. Reports to Team Leader Assessment and Planning.
