Based at the Victorian Pride Centre in St Kilda, with work across community settings and other BHN locations as requiredFull Time, PermanentAs a not-for-profit organisation, Better Health Network supports people and families at different stages of life and works with communities to strengthen health and wellbeing.We are seeking an experienced Young Person's Support Worker to provide intensive case management, outreach and practical support to young people between the ages of ***** who reside in low-cost housing, are homeless, at risk of homelessness and/or are working in the sex industry in the City of Port Phillip.About the roleResourcing Health and Education in the Sex Industry (RhED) is a specialist program within BHN's Resilient Communities unit.
RhED provides health promotion, education, advocacy, and support to people working in the sex industry across Victoria.
The program delivers a range of outreach and centre-based services designed to improve health, wellbeing, and safety, while reducing stigma and barriers to care.As a Young Person's Support Worker within BHN's RhED team, you'll provide individualised, trauma-informed and strengths-based support across community, accommodation and service settings.Your responsibilities will include:Providing intensive case management and individualised support to young people with complex needsUndertaking assertive outreach across community and supported accommodation settingsCompleting client assessments, intervention planning, risk management and safety planningAdvocating for clients and supporting access to housing, health, counselling, legal, financial,



education and employment servicesResponding to clients experiencing crisis or requiring immediate supportHelping clients develop practical living skills that support long-term housing stabilityBuilding effective referral pathways and partnerships with community, health, housing and government servicesMaintaining accurate client records and contributing to program evaluation and quality improvement activitiesAbout youYou'll be an experienced community services professional who can build respectful relationships with young people, work confidently in complex situations and deliver inclusive, non-judgemental support.To be successful, you'll bring:A tertiary qualification in health, social work or a related discipline, with professional registration where relevantDemonstrated case work or case management experience supporting marginalised young people with complex needsExperience applying trauma-informed, strengths-based and culturally safe approachesWell-developed assessment, intervention, risk management and safety planning skillsKnowledge of crisis intervention and experience responding appropriately to crisis situationsAn understanding of prevention, health promotion and harm-minimisation principlesStrong written and verbal communication,
