Alpine Health is seeking a part time Care Partner in Victoria to manage care plans and coordinate services for SAH participants under the Aged Care Quality Standards.
This role involves working with participants, informal carers, and families to plan, implement and monitor care aligned with individual goals.
The Care Partner will ensure dignity, risk-aware decision making, and regular monthly touchpoints, with occasional home visits and collaboration with the clinical care team.
