The Ted Noffs Foundation is seeking a dedicated Case Manager to join the Calm Continuing Care program in NSW.
You will deliver case management for Palm participants and those transitioning from Palm, conducting thorough assessments and developing Life Management Action Plans with clients.
You will provide ongoing support, coordinate with a multidisciplinary team, and help clients achieve educational and vocational goals while applying harm minimisation principles in a therapeutic community
