About My Rehab Team
My Rehab Team is a growing multidisciplinary allied health organisation delivering mobile, community and clinic-based services across Brisbane.
While this position is primarily mobile and community-based, Medsana Medical Clinic, our Eight Mile Plains facility, provides a central base and opportunities to collaborate with our multidisciplinary allied health and medical team.
Our team includes physiotherapists, occupational therapists, speech pathologists, exercise physiologists, podiatrists, dietitians, nurses, social workers, therapy assistants and support coordinators.
Our approach is practical, collaborative and outcome-focused.
We help clients improve their independence, participate in everyday life and remain safely supported in their homes and communities.
About the role
You will deliver individualised physiotherapy services primarily in clients' homes and community settings across Brisbane.
Depending on your interests and experience, your caseload may include:
Neurological rehabilitation
Mobility, balance and falls prevention
Functional rehabilitation and reconditioning
Chronic disease and pain management
Disability-related supports
Rehabilitation for older people
Equipment and mobility recommendations




Exercise prescription and home programs
Hydrotherapy
Collaboration with therapy assistants and other clinicians
Clients may receive services through the NDIS, Support at Home, Medicare, private funding and other funding arrangements.
We will work with you to develop a manageable caseload that reflects your experience, location and clinical interests.
Key responsibilities
Conduct comprehensive physiotherapy assessments
Develop practical, individualised and goal-directed treatment plans
Deliver physiotherapy in clients' homes and community settings
Support clients to improve their mobility, safety, independence and quality of life
Monitor progress and adjust treatment programs as clients' needs change
Prepare accurate, timely and high-quality clinical notes and reports
Communicate effectively with clients, families, support coordinators, referrers and other providers
Collaborate with our multidisciplinary allied health and medical teams
Contribute to safe, evidence-based and person-centred outcomes

About you
You will be a strong fit for our team if you are:
A qualified Physiotherapist with current AHPRA registration, or eligible to obtain registration
A new graduate ready to develop your skills within a supportive environment, or an experienced clinician seeking greater flexibility and autonomy
Compassionate, reliable and committed to high-quality care
Comfortable working independently while remaining connected to a broader team
An effective communicator with clients, families and other professionals
Organised and committed to accurate and timely documentation
Focused on meaningful functional outcomes and person-centred care
Experience in neurological rehabilitation, disability, aged care, community health or mobile physiotherapy is preferred but not essential.
New graduates are encouraged to apply
