This is a Community Worker role supporting elderly clients' social participation, independence, wellbeing, and connection with community resources.
The main duties are community participation, social support, wellbeing support, client coordination, family liaison and service connection.
This is not a primarily personal care role and is not mainly focused on showering, toileting, heavy personal care, medication administration, nursing duties, manual handling as the primary function, or routine domestic cleaning as the core duty.Key responsibilitiesAssess clients' social, emotional and community support needs through regular communication and observationDevelop and assist with individual community participation, lifestyle and wellbeing support plansOrganise and accompany clients to community activities, social outings, cultural events, appointments and interest-based programsEncourage elderly clients to maintain independence, confidence, routine, social connection and community involvementProvide emotional support, companionship and practical guidance to clients who may feel isolated or vulnerableLiaise with families, care coordinators, community organisations and service providers to ensure clients receive appropriate supportIdentify suitable local services,



community resources, welfare programs and social support options for clientsMonitor clients' progress, participation and wellbeing, and report concerns or changes to relevant supervisorsMaintain accurate client notes, activity records and basic progress reports in line with organisational proceduresPromote culturally sensitive support for clients from diverse language and cultural backgroundsAbout youRelevant qualification in Community Services, Aged Care, Disability, Social Work, Welfare, or a related fieldPrevious experience in community services, aged care lifestyle support, social support, welfare support, or disability/community programsStrong communication and interpersonal skillsAbility to work with elderly clients, families and multicultural communitiesGood record-keeping and reporting skillsCompassionate, patient and culturally respectful approachDriver Licence and First Aid Certificate preferredBilingual or multilingual ability will be highly regardedAbout usAusta Care is a multicultural aged care and community services provider supporting elderly clients and culturally diverse communities across Australia.
