Liquid Personnel is recruiting an experienced Social Worker – Learning Disabilities Team for its client's Learning Disabilities Team based in Liverpool.
This is a full-time agency role offering £***** per hour, supporting adults with learning disabilities to achieve positive outcomes through person-centred assessment, support planning, and safeguarding practice.

What will your responsibilities be?
In this role, you will:
Undertake strengths-based assessments to identify eligible care and support needs.
Develop and co-produce person-centred care and support plans with adults, families, and partner agencies.
Manage a diverse caseload of adults with learning disabilities, ensuring timely intervention and effective outcome-focused practice.
Complete reviews and monitoring of support plans to ensure needs continue to be met.
Work within safeguarding procedures to protect adults at risk and promote wellbeing.
Contribute to hospital discharge planning, transition work, and multi-agency assessments where required.
Maintain accurate and up-to-date case records in line with legislation, departmental policy, and professional standards.
Promote independence, choice, control, and social inclusion for adults with care and support needs.
Represent the department professionally and comply with national legislation, guidance, and local procedures.
Qualifications & Experience:




To be successful in this role you must have:
Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
