Provide care across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.Centered Care: Assess, develop, and evaluate nursing care plans collaboratively, focusing on client goals and independence with support from Allied Health clinicians.What you need to be considered for the role?
Understanding of community nursing and primary health care principles, including working within a consumer-directed care frameworkAt least 2 years of experience in community, acute, hospital or aged careExperience working independently or as part of a teamStrong organizational skills and attention to detailProficiency with electronic devices and client management systems (smartphones, tablets, laptops, email, etc.)Current Registration with AHPRA, Police check, working with children check and CPRExperience providing advanced clinical care, including chronic disease, medication, palliative care, catheter management, stoma care, wound care, and diabetes managementAbout Usintegratedliving is an industry-leading provider known for its person-centered approach to care.
