Youth on Track is an early intervention program supporting young people aged ***** years who are at risk of becoming involved in the criminal justice system.As a Youth Support Case Worker, you will work directly with young people and their families to provide culturally appropriate, strengths-based and holistic support.
You will assist young people to address the issues contributing to antisocial and/or offending behaviour while building on their strengths, prosocial behaviours and connection to culture, family and community.Supports are tailored to individual needs and may include case management, service coordination, referrals, mentoring, cultural activities, recreation, evidence-based programs and individual or group-based support.Key ResponsibilitiesWork in partnership with young people, families, community and service providers to identify concerns and develop appropriate supportsProvide culturally protected, client-centred and strengths-based case managementSupport young people and families to identify their own goals, concerns and solutionsCoordinate access to relevant services and maintain clear referral pathwaysFacilitate access to cultural, social, educational,
