Real Community Services – Browns Plains QLDAn opportunity has become available to step into an established caseload while contributing to a growing and supportive team.At Real Community Services, we are guided by our values of diligence, respect and innovation, and work alongside people with disability, their families and supports to deliver practical and thoughtful services within their communities.About the RoleAs a Support Facilitator, you will work closely with individuals, teams and families across the Brisbane – Gold Coast region to facilitate supports, navigate the National Disability Insurance Scheme (NDIS), and ensure services are responsive and aligned with individual needs.This is a full time Hybrid role with flexible WFH options.Key ResponsibilitiesManage an established caseload with professionalism and careWork alongside participants and families to understand needs and prioritiesFacilitate services by implementing strong and consistent teams / individual supportsMonitor National Disability Insurance Scheme plan utilisationPrepare progress and review reports ensuring timely service review and stakeholder collaboration prior to renewal of plan periodBuild strong relationships with stakeholders and providersMaintain accurate and compliant documentation in our CRM and associated industry compliance portals (such as for example NDIS Commission Reporting)About YouYou bring empathy, professionalism and strong organisational capability, with confidence working independently and as part of a team.Experience in disability services,



support coordination/facilitation or case managementStrong working knowledge of the National Disability Insurance Scheme framework, including Supported Independent Living (SIL), Specialist Disability Accommodation, activities of daily living supports and community access.
