long-term patient relationshipsStable permanent role in a well-established practiceAbout the RoleKey responsibilities include:Developing and reviewing Chronic Condition Management PlansConducting health assessments and preventative health activitiesProviding patient education and lifestyle supportMonitoring patient progress and coordinating follow-up careWorking closely with GPs, specialists and allied health providersIdentifying patients eligible for chronic disease management servicesMaintaining accurate clinical and Medicare documentationSupporting quality patient outcomes through collaborative careAbout YouTo be successful, you will have:Current AHPRA registration as an RN or EENExperience in General Practice or Primary Healthcare (highly regarded)Knowledge of Chronic Condition Management and preventative healthStrong communication and organisational skillsA patient-centred approach and genuine passion for healthcareAbility to work independently and as part of a team
