We are seeking experienced and confident Disability Support Workers in Ararat / Ballarat to support participant in palliative care.
You will be working closely with clinicians, and support coordinators to implement care plans that prioritise safety, dignity, and wellbeing.Travel hours will be reimbursed to staff if travel time is more than 30 minutes to work.Key responsibilitiesProvide hands-on daily support and personal care to participant.Support participants with medication prompts, emotional regulation, and daily routinesMaintain clear and accurate documentation (shift notes, incident reports, etc.)Work collaboratively with the wider multidisciplinary and clinical team.About youMinimum 6 months experience in palliative care.Certificate III or IV in Disability, Individual Support, or Mental Health (or equivalent)Current NDIS Worker Screening Check & Working with Children CheckCurrent First Aid and CPR CertificationValid Driver's Licence and reliable transportStrong interpersonal skills, reliability,
