What You'll Be Doing
Partnering with NDIS participants to implement their plans effectively
Assisting clients to understand their funded supports and make informed choices
Coordinating a network of supports, including providers, health professionals, and informal networks
Advocating for participant needs and ensuring services align with their goals
Keeping thorough documentation and maintaining compliance with NDIS requirements
Identifying and managing risks,



and supporting participants through plan reviews
Staying current with NDIS practices and changes
Participating in reflective practice, supervision, and team development activities
What You Bring to the Role
Experience in Support Coordination or a related support role
Knowledge of the NDIS and a strong commitment to the rights and goals of people with disability
A qualification in Social Work, Psychology, Allied Health, or Community Services (or similar)
Strong time management and communication skills
Ability to work independently and as part of a team
Capacity to manage a caseload and travel within Brisbane, Gold Coast, Ipswich & surrounding areas
Valid Driver's Licence and reliable, insured vehicle
Current First Aid and CPR
NDIS Worker Screening Check & National Criminal History Check (or willingness to obtain)
