Blossoming Support Service is a community-based care provider committed to delivering respectful, reliable and person-centred support to clients in their homes and in the community.Due to increasing demand for our services, we are seeking an experienced and compassionate Aged or Disabled Carer to join our team on a full-time basis.Key responsibilitiesProvide personal care, including assistance with showering, dressing, grooming and toiletingAssist clients with mobility, transfers and the secure use of mobility equipmentProvide companionship, emotional support and reassuranceAssist with meal preparation, feeding, nutrition and hydrationPerform light household duties, including cleaning, laundry and changing bed linenSupport clients with shopping, appointments, recreational activities and community participationProvide medication assistance or reminders in accordance with care plans and company proceduresFollow individual care plans, behaviour support plans and risk-management proceduresObserve and report changes in clients' health, wellbeing, behaviour or support needsMaintain accurate progress notes, service records and incident reportsPromote client choice, independence, privacy,



dignity and cultural safetyFollow infection control, manual handling, WHS and safeguarding proceduresCommunicate effectively with clients, families, care coordinators and healthcare professionalsQualifications and experienceCertificate III in Individual Support (Ageing or Disability), or an equivalent relevant qualificationAlternatively, substantial relevant experience equivalent to the required qualificationAt least 1 year of relevant experience as an aged care worker, disability support worker, personal carer or similarCurrent First Aid and CPR certificatesCurrent National Police CheckNDIS Worker Screening Check where requiredValid Australian driver licence and access to a reliable insured vehicle where community travel is requiredStrong communication, documentation and interpersonal skillsAbility to work independently and as part of a multidisciplinary teamFull-time position – 38 hours per week Salary: $79,500 per annum + superannuationAvailability to work on rotating rosters, including some evenings, weekends and public holidays, may be required.
