HiTalent Recruitment is seeking a Care Coordinator to join a well-respected and reputable not-for-profit organisation in Wagga that is funded to deliver inclusive adult mental health and AOD services to the community. This role is casual, full time, Monday - Friday business hours.

Are you a compassionate and skilled Care Coordinator with a desire to deliver high-quality, recovery-orientated, and person-led support services?

n

In this position, you will provide effective service delivery, ensuring case management, assessment and referral, education, group facilitation and support.

n

Reporting to the Service Manager, you will be responsible for:

n

Provide effective day-to-day service delivery through education, interventions, group work, assessment, and referrals.

n

Develop, implement, and monitor individual treatment plans, ensuring all case management documentation, records, and reports are accurate and compliant.

n

Acquire and maintain robust knowledge of referral pathways and support options available to participants.

n





Work collaboratively and professionally with other services to provide holistic care.

n

Demonstrate commitment to continuous improvement, innovation, and best practice in service delivery.

n

Undertake ad hoc duties as required by management.

n

Contribute to program development and continuous improvement initiatives.

n

Uphold organisational values and policies while maintaining a commitment to quality service delivery.

n

To be successful in this role, you will require:

n

A tertiary qualification in Social Work, Counselling, Psychology, or Community Services, or relevant work experience.

n

Experience supporting people impacted by substance use and mental health conditions.

n

Knowledge of mental health, AOD, community services, and health sectors

n

Excellent communication skills and the ability to collaborate effectively within a multidisciplinary team and build relationships.

n

Ability to work independently and in a team
