The role focuses on enhancing client independence, wellbeing, and community participation through effective delivery of services such as personal care, domestic assistance, social support, transport, and respite while contributing to a secure, collaborative, and compliant work environment.Key Duties/ResponsibilitiesWithout limiting the general scope and application of this Duty Statement, the duties include:Direct Client SupportProvide individually negotiated services under the CHSP, HCP, Brokerage frameworks for clients in areas such as:Social SupportCommunity TransportDomestic AssistancePersonal CareRespiteGarden MaintenanceDrive Valmar vehicles and operate machines, where licenced and where requiredAt all times, and in all locations, work with the clients in a positive and age-appropriate manner in line with the Valmar Code of ConductEnsure the concept and principles of wellness, enablement,



and restorative care are understood and moulded to suit the individual circumstances of clients and service settingsAssist with medication reminders and prompts (as per care plan and scope of practice)Assist clients with mobility and use of mobility aids (e.g., walking frames, wheelchairs)Provide companionship and emotional support to reduce isolationSupport clients in engaging with the community and meaningful activitiesAssist with errands such as grocery shopping, pharmacy runs, and appointmentsPromote independence through daily living skills and supportive conversationCompliance, Monitoring & ReportingEnsure services are delivered as per funding guidelines and regulatory frameworksEnsure clients, carers, and other support persons have avenues for providing input into the service delivery arrangement and negotiationsMaintain client documentationComplete Incident reports as requiredParticipate in the Valmar Performance Appraisal processSafety, Risk & Vehicle ResponsibilitiesPromote and encourage all aspects of safety within the home and the communityWork within Valmar WHS guidelinesConduct vehicle maintenance checks on fleet vehicles as requiredUse safety procedures when transporting clients, mobility equipment, and goodsIdentify and report risks or hazards in the client's home (e.g.,



fall risks, unsafe conditions)Monitor client wellbeing and report significant changes in physical, emotional, or cognitive status to supervisorsEnsure heating/cooling, hygiene, and general home safety standards are metAssist with infection control practices (e.g., PPE use, hygiene practices)Teamwork & CommunicationContribute to team-based activities, meetings, and decision-makingCommunicate effectively with clients, families, carers, and external stakeholdersLiaise with health professionals and community services when requiredDemonstrate professionalism, empathy, and respect at all times, adhering to the organisation's Code of Conduct and policiesOther:Complete any reasonable requested tasks to support and enhance client-centred careAbout You:Successful applicants must be willing to undertake a NDIS Worker Screening Check and hold an Australian Drivers Licence.
