Job Description

Anglicare Victoria is recruiting a Senior Case Manager for the KEYS residential program based in Dandenong. This full time role (SCHADS 6) leads intensive case management and coordinates care teams for up to eight young people across two homes.

You will collaborate with partner services, maintain case notes and assessment plans, and support effective transition planning. The KEYS program focuses on culturally protected, team-oriented practice.
