Practitioner/Aboriginal
Health Worker to join the Primary Care team. You'll deliver versatile, holistic, culturally safe health care to Aboriginal and/or Torres Strait Islander people, focusing on prevention and management of chronic disease and continuity of care within custody to support transition back to community.
